Once you schedule an appointment, we will send you our digital form via email or text. If you’d prefer to fill out this form at our office, we ask that you arrive 10 minutes early to your first appointment.
What information will I need to fill out?
- Your complete health and medical history, including current medications and supplements, and the names of your doctors. Don’t worry if you are unsure about your medical history or if it is extensive – we can always reach out to your doctor directly for clarification or detailed information.
- Dental insurance details, as we accept most PPO insurance plans, and information about previous dental treatment.
- Your preferred contact details and any communication or accessibility needs.
If you need help completing our digital form
Let the office know before your visit if you need assistance reading, accessing or completing the digital form. We would be happy to assist you with it, whether in-office or over the phone.
